Opening a Health Savings Account

Employers

If you are an Employer, please contact us so that we may provide you with Health Savings Accounts for your Employees. You have a choice to contact us by:

Individuals

If you are an Individual and you would like to enroll in ISU's Health Savings Account, please use the Open An HSA Button on this page.

To open a Health Savings Account you are required to meet the following criteria:

• You are covered by a High Deductible Health Plan
• You are not covered by another health plan including Medicare
• You are not claimed as a dependent on another person's insurance policy
• Your insurance deductible amount meets the minimum deductible requirements

Please have the following information available when you begin Health Savings Account enrollment:

• Health Insurance Card
• The last four digits of your dependents' Social Security Number
• The names, addresses and Social Security Number of your Beneficiaries
• Checking Account Information (located at the bottom of your check)
